While on-duty, failed to properly investigate off-duty Police Officer for operating a vehicle while under the influence of or impaired by alcohol.

While on-duty, wrongfully made or caused incomplete or inaccurate entries in Department records.

While on-duty, failed to report immediately to the Patrol Supervisor and the platoon commander any unusual crime, occurrence or condition.
