On May 12, 2014 Jamie [redacted] requested that his wife [redacted] call an ambulance because he was feeling anxious. [redacted] asked her friend, [redacted] to make the call and he did. PO Jenze Perez and PO Malachi McKenith of the 40th Precinct arrived at the location without an ambulance. [redacted] opened the apartment door and both PO Perez and PO McKenith pushed her aside and entered (Allegations A, B, C and D). [redacted] closed the bedroom door and stated that he wanted an ambulance and not officers. Both officers broke down the bedroom door (Allegations E and F). [redacted] was thrown onto the bed and punched more than ten times, causing bruising and lacerations to his head and face (Allegations G and H). Sgt. Angel Delgado arrived at the location after [redacted] was handcuffed, held up a taser to [redacted] chin and stated, "If you move, I'll shock you" (Allegation I). Read report for details.
