Description: On January 31, 2016 decedent was found unresponsive in his holding cell and his death was attributed to suicide. His cell was not monitored by police officer White or any other police officer, as required or assigned to such task. The cell had a clear plastic glass window which allowed police personnel at the front desk to view/or look directly into the holding cell where decedent was held.
