  1. Guilty: On twelve (12) occasions, failed to report for his scheduled tour, and failed to submit lost time requests for said occasions, resulting in his receiving approximately one hundred and three (103) hours of compensation he was not entitled to
  2. Guilty: On twelve (12) occasions, failed to supervise two subordinates under his supervision when he failed to appear for his scheduled tour of duty
  3. Guilty: On fifteen (15) occasions, failed to sign in at the start of his tour and out at the end of his tour in the command log as required
  4. Guilty: Failed to safeguard his activity log resulting in its loss, and failed to notify the Department about its loss
